While this is mostly theoretical, the hash functions can fail and it is
better for the upper layer code to explicitly check for such failures.

Clean up eap_sake_parse_add_attr() design by passing in pointer to the
payload of the attribute instead of parsing these separately for each
attribute within the function.

In addition, start ordering header file includes to be in more
consistent order: system header files, src/utils, src/*, same
directory as the *.c file.
